i held mine with my hand...and then used muscle to turn the screw some people loctite it due to handlebar vibration that might be why its taking effort, i havent and didnt find any problems(so far haha)

I'm hoping you went with some nice R&G bar end sliders?

If you have alot of trouble you can use something like a strap wrench or channel locks/vise grips and rags to keep from scratching the bar ends.

If they are installed correctly there is a lip on the bar end that indexes with the weights in the handlebars that should keep it from rotating if the bar end is fixed somehow and the screw is turned.

You might find it easier to remove the whole assembly. Peel back the grips at the bar end side and push down on the clip that holds the internal handle bar weight inside the handlebar. Then pull on the weight and disassemble the assembly off the bike. You might need to spray some lubricant in the handlebar to get the assembly out.
